This has been pieced together over both JTHM and IFS.

Devi’s world is over dramatized from a fairly pessimistic and unforgiving point of view but is a modern style city, circa mid 1990’s of western America. The population is stupid and gullible and speaks to different and likely offensive stereotypes. There’s not a lot of free reason or logic - Johnny gets away with murdering a horde of people and never getting caught because of this (One could also count that towards the supernatural edge of this particular reality). Corporate greed, bad slogans calling to the baser natures of man, the terror of stale bagels and fangirls to an extreme are all an accentuated feature of her reality.

The population of Devi's world emits a negatively charged sludge. Think Ghostbusters 2. This sludge, fueled by the ever growing negative emotions is siphoned away but objects called Waste Locks. Waste Locks are usually mundane items, but without them, the people of the world would be driven to do unspeakable acts in order to keep the balance. Waste Locks store the sludge in a pocket of the Astral Plane until it's reached capacity. Once that happens, reality combusts and is basically rebooted, with another waste lock set to do the job all over again.

One waste lock, a wall in a house, managed to gain sentience and created a new job for the Universe. Flushers. Flushers are guardians to the waste locks and filter sludge to the lock as they go about their lives. Johnny was the first Flusher chosen and his personal insanity lead to the eventual manifestations of the Dough Boys, named so by the two styrofoam Pillsbury Doughboys that Johnny talked to. The Doughboys consumed Johnny's memories to transfer to the styrofoam and when the reboot occurred, it was them who chose Devi to be the new flusher. The Doughboys were joined into one entity that ate in Devi's mind as they tried to do the same that they had to Johnny to her.

Since the physical manifestations are chosen by the flusher, Devi unwittingly gave them a unfinished painting of hers. A painting of a little doll that was her defining piece of work at the time, that she had poured her soul into. The doll was later named Sickness.
